The Boca Raton Comprehensive Cancer Center ("BRCCC") will permanently close
its business operation as of 5:00 p.m. on Friday April 9, 2010.

Prior to closure, patients may request copies of their medical records by either:

• Calling BRCCC (561) 883-7555, or

• Visiting the center located at 21020 State Road #7
Boca Raton, Florida 33428.

After April 9, 2010, patients requesting medical records should contact (561) 883-7555.
